Clinical Presentation of Deep Venous Thrombosis (DVT)

Signs and Symptoms of DVT include the following:

  • Unilateral leg pain, warmth, and/or swelling
  • Dilated superficial veins

Risk factors for DVT:

  • History of immobilization or prolonged hospitalization
  • Recent surgery or trauma
  • Obesity
  • Previous venous thromboembolism (VTE)
  • Malignancy
  • Use of oral contraceptives or hormone replacement therapy
  • Pregnancy or postpartum status
  • Stroke with hemiplegia or immobility
  • Age > 65 years
  • Family history of venous thromboembolism (VTE)
  • Heart failure
  • Inflammatory bowel disease

Natural History of Venous Thromboembolism (VTE)


Most deep vein thromboses (DVTs) begin in the calf, and most resolve spontaneously.

Thrombi confined to the calf rarely cause leg symptoms or symptomatic pulmonary embolism (PE).

A prothrombotic stimulus increases the probability that a calf DVT will extend to the proximal veins and subsequently cause a pulmonary embolism.

At the time of diagnosis of symptomatic pulmonary embolism, most patients have thrombosis at both sites: deep venous and pulmonary.

Approximately 10% of PEs are rapidly fatal.

Of diagnosed PEs, approximately 50% are associated with right ventricular dysfunction.

Surgeries at High Risk for Venous Thromboembolism


• Abdominal or pelvic surgeries for cancer
• Major trauma
• Craniotomy/spinal surgery for malignant disease
• Spinal surgery with anterior approach
• Thoracic surgeries
• Major orthopedic surgery

Significance of Deep Venous Thrombosis


Symptomatic perioperative venous thromboembolism is a frequent complication in patients with a history of deep vein thrombosis (1).
Incidence of DVT in perioperative population:

  • 3.6% of patients presenting for percutaneous kyphoplasty (2)
  • 6.4% of patients with ankle fractures (3)
  • 3.7% of patients with lower extremity fractures, and 6.2% of post-operative patients (4)
  • 4.4% of patients presenting for gastric cancer surgery, and 7.2% of post-operative gastric cancer patients (5)
  • Venous thromboembolism is a factor in 5% of patients that die within 30 days postoperatively (6)

Significance of Venous Thromboembolism (VTE)


VTE is a leading cause of preventable hospital death in the USA.

VTE is the fifth most frequent reason for unplanned hospital readmissions after surgery, overall.

Blood clots affect as many as 900,000 Americans each year leading to approximately 100,000 premature deaths.

Introduction


Point of care ultrasound can determine the presence of deep venous thrombosis (DVT) in the lower extremity of patients in critical situations.

DVT evaluation may be useful for anesthesiologists, emergency physicians, or intensivists, or other physicians. There is utility of DVT evaluation in clinics, emergency departments, preoperative evaluation, post-operative period, or when patients are in critical condition
