Basic FATE & Basic Lung Ultrasound Course and Advanced FATE Course

(Focus Assessed Transthoracic Echocardiography)

Overview:

Transthoracic echocardiography (TTE) is a tool commonly used by cardiologists to answer complex clinical questions regarding a patient’s cardiovascular health; however, few physicians without formal training in cardiology utilize a focused TTE exam for its other potential benefits. Focused TTE has been shown to be an important tool in the hands of physicians in the perioperative setting (for pre-surgical assessment and intra/post-operative management) to supplement clinical evaluation and optimize cardiopulmonary resuscitation.

TTE offers real-time assessments, which augments its utility as a monitoring tool as it provides a more accurate and detailed cardiovascular assessment than physical examination alone. In addition to the benefits of TTE as a diagnostic tool, TTE can also be taught successfully to non-cardiology physicians in a short period of time.

Despite the growing awareness among anesthesiologists and intensivists regarding the benefits of focused TTE, there are few courses available and a lack of existing standardized curriculum and proper equipment to adequately train these physicians in order to incorporate this tool into their daily practice. Upon completion of this Basic FATE course, physicians will be able to utilize a focused TTE exam in their daily practice as a means to supplement their standard clinical evaluation as well as answer simple yet important clinical questions in order to optimize cardiopulmonary resuscitation.

Target Audience:

This activity is targeted to anesthesiologists, emergency medicine professionals, internists and critical care physicians.

Learning Objectives:

UMass Memorial Healthcare and UMass Medical School Professional Education activities are intended to improve the quality of patient care and safety.

At the end of the Basic FATE and Basic Lung Ultrasound seminar, participants will have learned to:
•Recognize and utilize the knobology of the machine in order to optimize imaging.
•Identify TTE images of each of the four FATE views (subcoastal 4-chamber, Apical 4-chamber, Parasternal Short Axis, Parasternal Long Axis) as well as interpret pleural images on both sides of the thorax.
•Recognize normal cardiac and pulmonary images versus pathology in an acutely ill patient.
•Perform a complete FATE exam (including all 4 views and pleural scanning) within a limited timeframe to mimic real life practice and interpret the resulting images.
•Interpret cardiac function (normal vs. abnormal) using Mitral and Tricuspid Annular Plane Systolic Excursion (MAPSE and TAPSE), Mitral Septal Separation (MSS) and Fractional Shortening.
•Perform a simple and systematic sonographic examination of the lungs which can be used in trauma, and emergency situations.
•Recognize normal lung tissue, and be able to diagnose and exclude pleural effusion, pulmonary edema, and pneumothorax.
•Distinguish which clinical circumstance call for the use of a focused TTE exam in order to supplement clinical practice and resuscitation.
•Recognize the times to request consultation when the clinical questions are beyond the scope of the physicians’ basic knowledge and training in FATE.

At the end of the Advanced FATE Seminar participants will have learned to:
·       Obtain extended 2-dimensional imaging views (used in cardiology and perioperative medicine)
·       Basic Doppler ultrasound principals
·       Advanced Doppler ultrasound for pressure estimation, cardiac output estimation and assessment of diastolic function
·       Advanced clinical scenario training

Instructions for Post Test and CME Credit:
In order to receive CME Credit and attend the on-site course at The University of Massachusetts Medical School, all participants must complete this online e-learning module. This e-learning module provides all of the educational material needed in preparation for the on-site event, allowing for minimal lectures and a full day of hands-on learning. Given the importance of the e-learning module, you will not be allowed to attend the on-site course without completing the e-learning module and achieving a passing score of greater than >90% on the post-test. You can take the test multiple times.
